Patient Centered Medical Home 

Patient Centered Medical Home (PCMH) model is designed to provide primary health care that is relationship-based, with the whole person concept in mind. The focus of this model is to provide quality accessible health care, by a team of professionals who know and understand your family and their unique needs. 

Your PCMH team can include: 

  • Physicians 
  • Nurse Practitioners 
  • ​Physicians Assistants 
  • Nurses 
  • Pharmacists 
  • Disease Managers 
  • Case Mangers 
  • Medical Technicians 

PCMH coordinates care across all elements of the healthcare system, including specialty care, hospitals, home health care, and community services.

Mission 

Deliver the highest quality, evidence based, patient centered care to enrolled patients through team-oriented processes, increased access, improved provider continuity, enhanced communications, and coordinated prevention, education and management of patients. 

Vision 

Ensure operational health and readiness of all military members and ensure clinical currency of team members.

You have the right to obtain care from other clinicians within the Primary Care Medical Home, or to seek a second opinion, and/or specialty care. Patients have a responsibility to participate in their own care. Self-management goals can help you manage your health!
